Obituary of Hazel Mae Tormohlen Bennett

Hazel Mae Bennett, RN, 84, passed away 9/11/04 following a lingering illness of several years and recent fall at the family home in Phoenix. Born in Holland, Indiana, 9/21/20, the oldest of three daughters to Harry John and Amelia (Werremeyer) Tormohlen. Graduate of Deaconess Hospital College of Nursing, Evansville, Ind. Moving to Phoenix in 1948, working at Good Samaritan and Phoenix Memorial Hospitals. Later as Executive Secretary and then Executive Director of the AZ Nurses’ Assn. for 25 yrs when she retired. Survived by husband of 53 years Jay C. Bennett; sons John Eugene and Robert Lewis (Shawna Kay); grandsons Tyler Jaskey and Cody Jay; sister Mabel Caroline (Guy) Crab; nephews Wayne Crab and Alan (Connie) Crab; grand nephews Jason and Joshua Crab; and numerous cousins living out-of-state. Memorial service to be held 2:00 PM, Thursday, 9/16/04, at Hansen Chapel, 8314 N. 7th St., Phoenix, 85020, 602-944-1561.
